consent to transport

The Undersigned certifies that they are the owner(s) or authorized agent for the owner(s) of the described vessel and gives/conveys to Captain Robert Carocari the care, custody, and control of the described vessel.

The undersigned further agrees to hold harmless, Captain Robert Carocari for any insured or uninsured occurrence, regardless of circumstance. The owner(s) or owner(s) agent further certifies that the described vessel is insured for the waters being sailed and that the registration/document is in order and current.

A current copy of the vessel’s policy with Captain Robert Carocari listed as additional insured will be required to be on the vessel before departure.
